If your shipment includes four or more unique samples or commodities, you’ll need to complete a CSV file instead of entering each item manually in the form.
This helps us process your request more efficiently and accurately, while avoiding issues with the online form’s capacity.
What You Need to Do:

Submit Your Shipping Request via the Web Form
Complete the shipping request form as usual, selecting ‘4 or more – Please submit a CSV file’ in the ‘Number of Sample Types’ field, then submit the form. -
Download the CSV Template
Use the appropriate template below based on your shipment type: -
Fill Out the CSV File
Complete all relevant fields for each unique sample or item. If you're unsure about any fields, reach out—we’re here to help. -
Email the Completed CSV
After submission of your shipping request, you’ll receive a confirmation email. Please reply to that email and attach your completed CSV file. This ensures we can match it to your request.
